We will provide you with an Encounter Document. It will contain all the codes related to your visit, as well as the information regarding the clinician with whom you met. Many patients submit the Encounter Document to their insurance companies and receive some reimbursement.
We encourage you to contact your insurance company directly to determine details of coverage and reimbursement. You should be aware that insurance companies often require a clinical diagnosis, treatment plans, treatment summaries, or copies of a patient’s entire record. We shall only provide this information if you sign a release form specifying who is to receive the information, type of information to be released, the purpose for which the information is being requested, and for what period of time the authorization is valid.
This agreement is being provided according to the Health Insurance Portability and Accountability Act. We will be happy to provide you with a copy of this agreement if you request one.
What is the cancellation policy?
In order to provide the best care for clients hoping to fill cancelled appointments, reminder calls will be made 24 hours before a scheduled appointment. To avoid being charged for your appointment, you must cancel by 10:00 am the full business day before the appointment. For Monday appointments you must cancel by 10:00 am on Friday. Appointments cancelled after the deadline will be billed at the full rate of the appointment.
Late cancellations due to unexpected illness or other emergencies will be evaluated on an individual basis by the clinician.
